How much does it cost?
In-Studio Physical Therapy:
Initial evaluations cost $175. Follow up appointments are $150. Physical Therapy appointments are approximately 50 minutes in duration. A good faith estimate can be provided if requested.
In-Home Physical Therapy:
Initial evaluations cost $200. Follow up appointments are $175. Physical Therapy appointments are approximately 50 minutes in duration. A good faith estimate can be provided if requested.
Does my insurance cover this?
Look at Mommy PT is fee for service provider. Although we do not bill insurance directly, we can provide you with a superbill (receipt) to self submit claims to your insurance for any out-of-network benefits .

Please note that if you have Medicare as a primary or secondary insurance, Medicare does not allow for it's beneficiaries to be treated by non-contracted physical therapists. Pilates is an option in this case.

What is your cancellation policy?
In-Studio Physical Therapy Cancellation Policy:
Cancellations made less than 24 hours before your scheduled session will incur a $75 late cancellation fee.
Failure to attend a scheduled appointment without any prior notice (“no-show”) will result in a $75 no-show fee.
By scheduling an appointment, you authorize Look at Mommy, PLLC to charge the applicable fee to the payment method on file.
In-Home Physical Therapy Cancellation Policy:
Cancellations made less than 24 hours before your scheduled session will incur a $100 late cancellation fee.
If no notice is provided, and the therapist arrives at your home as scheduled, the full cost of the reserved appointment will be charged. This fee applies regardless of the reason you are unable to participate in the session at the scheduled time.
By scheduling an appointment, you authorize Look at Mommy, PLLC to charge the applicable fee to the payment method on file.
Why is there a cancellation fee?
We understand that life happens—schedules change, emergencies come up, and sometimes appointments need to be adjusted. At the same time, physical therapy works best when you attend consistently, and your scheduled appointment is reserved just for you.
When an appointment is canceled late or missed, it means another patient who may have needed that time couldn’t be seen. Our therapists prepare in advance for your session, and holding that spot is part of providing you with the best care possible.
For these reasons, we have a cancellation fee. It helps us:

Ensure fairness for all patients who are waiting for available appointments
The cancellation/no show fees for In-Home care are higher because more of the therapist's time is dedicated to this appointment.
We don’t like charging this fee. But having a clear policy in place allows us to provide consistent, high-quality care for everyone with respect to our therapist's time.

What is your cancellation policy for Pilates?
We require a minimum of 24 hours’ notice for any appointment cancellations or scheduling changes. Notice must be provided at least 24 hours prior to the scheduled start time of the session.
